Vaccination  and physicals clinic

FCCPS is partnering with Fairfax County Health Department and Kaiser Permanente to offer free physicals and immunizations to students. They will be provided by licensed health care providers from the Fairfax County Health Department and Kaiser Permanente.

All students who are entering  public school in Virginia for the first time, regardless of age or grade must have a Virginia School Entry Physical, state-mandated vaccinations, and a completed school health entry form.

Rising seniors must have the meningooccal vaccine and rising 7th grade students are required to have the Tdap vaccine.

Student(s) will have the opportunity to receive a physical examination, and needed vaccinations. The school health entry form will be completed by a healthcare provider to use for school registration.  

This event is open to all families in Falls Church City Public Schools.

  • Diphtheria, Tetanus, & Pertussis (DTaP, DTP, or Tdap). 

  • Hepatitis A (HAV) Vaccine. 

  • Hepatitis B (HBV) Vaccine. 

  • Human Papillomavirus (HPV) Vaccine. 

  • Measles, Mumps, & Rubella (MMR) Vaccine. 

  • Meningococcal Conjugate (MenACWY/MCV4) Vaccine. 

  • Polio (IPV) Vaccine. 

  • Varicella (VAR/Chickenpox) Vaccine
